At Groth Dental, we believe your oral health is too important to put off. Our membership plan was designed specifically for patients without dental insurance who want access to high-quality preventive care at a straightforward, predictable cost.

Program Guidelines
Before enrolling, please review the following plan details:
The membership plan cannot be combined with another dental membership or dental insurance plan
All membership fees are nonrefundable
No refunds will be issued if a member chooses not to utilize their benefits
Your portion of any treatment cost is due on the date of service
Membership is valid for one year from the date of premium payment
Membership renews automatically at the end of each term
A $75 reinstatement fee applies if your membership lapses
This plan is not insurance and does not qualify as a health plan under the Affordable Care Act
